Due to "no shows and same day cancellations" we now require that the "Self Pay" fee is due at the time of scheduling your appt.

Functional Wellness Membership $89/mo FOR THE INDIVIDUAL or $1450/year PER INDIVIDUAL (Enrollment fees + monthly fee paid in advance)

Functional Wellness Membership plans will have a one time Enrollment Charge $464 (initial visit fee + 1st month membership)

Each membership Includes:

 (1) Functional Medicine Visit (40 min) every 3 months.
